Our Financial Policy

Karmazin Dental Financial Policy

Here at Karmazin Dental we are committed to providing our patients with an exceptional experience.  Our focus is delivering the highest quality of comprehensive dental care matched up with five-star customer service and up-front communication.  The professional fees associated with dental care are an integral part of the patient experience.  We have defined our financial policy in efforts to help our patients understand what they can expect from us and what we expect from them.

 

Payment for services is due on the date service is provided unless previous arrangements have been set up with Karmazin Dental.  We accept cash, personal checks, debit card, HSA, Mastercard, Visa, Discover, and American Express.  Outside financing is available through Care Credit upon request and approval.

 

Karmazin Dental is out of network (non-participating) for all dental insurance companies.  As a courtesy to our patients, we will file all dental claims and accept assignment of benefits when allowed.  In order for us to be able to provide this service, it is the responsibility of the patient to provide Karmazin Dental with accurate dental insurance information.  We ask that all patients familiarize themselves with their specific insurance coverage as plan benefits can vary greatly.  Additionally, dental insurance plans have a wide variety of rules, restrictions, and limitations that our office may not be aware of.  As your dental care provider, our focus and our relationship is with you, the patient, not with your insurance company.  All incurred charges are ultimately the responsibility of the patient, regardless of insurance estimates or coverage.  Once insurance has completed a claim, a statement will be sent to the patient for any remaining balance and will be due upon receipt.  If your insurance company has not made a payment within 60 days, from the date of service, the unpaid balance will be expected to be paid by the patient (responsible party).

 

Returned checks and balances older than 60 days may be subject to collection fees and finance charges at the rate of 1.5% per month (18% annually).  Additionally, we ask that if for any reason you are unable to keep a scheduled appointment, please notify Karmazin Dental at least 24 hours prior to the appointment time.  If a patient fails to show up for an appointment without adequate notice, we reserve the right to assess a fee for that broken appointment.  The assessed fee could be up to 25% of the appointment's originally estimated cost.
